Finding Out the Cost of Covered Dental Procedures

Plan members receive a copy of the dental fee schedule with their membership materials approximately two weeks after joining. Members can also call their Plan dentist or Customer Services for cost details on specific procedures.

Moving or changing Plan dentists

ASignature Wellness Plan membership can be used nationwide where there is a of network providers. With this dental savings plan, members are free to visit any Plan provider. Members aren’t required to call about switching to a new provider.

Renewing membership

Plan membership is automatically renews and membership fees is automatically billed by the method and frequency of payment select on the enrolment form.
